All attendees are requested to fill in and submit the
V PanAmerican Workshop Registration
Form together with payment by Thursday,
April 15, 2004 for Early Registration. Any
registration received after Thursday, April 15, 2004 will be subject
to Regular Registration fee. If it becomes necessary to cancel your
attendance to the conference, you are required to submit a written statement,
name, address, phone number and email. Cancellations
before Friday, April 30, 2004 will be charged a process fee of 25% of
registration payment. Refunds will not be issued after April 30, 2004.
